No Medical Exam Life Insurance

Five years ago, almost every life insurance application meant a paramed visit, a blood draw, and a 4–6 week wait. Today, the majority of healthy adults under 60 can get approved in minutes with no needles involved. Here's how no-medical-exam coverage actually works in 2026 — and where it fits.

What "no medical exam" really means

The carrier still underwrites you — they just use data instead of a physical exam. That data typically includes:

  • The MIB (Medical Information Bureau) record
  • Prescription drug history
  • Motor vehicle records
  • Public records and credit-derived risk scores
  • Your application answers

It's called "accelerated underwriting" in the industry. The American Council of Life Insurers now reports a majority of policies under $1M are issued this way.

Types of no-exam policies

  1. Accelerated underwriting (true rates). If you qualify, you pay the same rate you'd get with a full exam. Often available up to $1M–$3M of coverage.
  2. Simplified issue. A short health questionnaire, quick approval, slightly higher rates. Common for $25k–$500k.
  3. Guaranteed issue. No health questions, anyone accepted. Low coverage limits, much higher rates, and a 2-year graded death benefit. Best for hard-to-insure applicants — see our final expense guide.

Who qualifies for the best rates without an exam

  • Age 18–60 (some carriers go to 65)
  • Healthy BMI and blood pressure
  • Non-smoker for at least 12 months
  • No recent serious conditions (cancer, heart attack, diabetes complications)
  • Clean driving record

When a full exam still makes sense

  • You need more than $1M of coverage
  • You have a borderline condition that an exam could clarify in your favor (e.g. controlled blood pressure)
  • You're older than the no-exam age limits
  • You want the absolute lowest possible rate and can wait 4 weeks

How fast is "fast"?

Best case: complete the application, get an instant decision, and e-sign your policy the same day. More common: approved in 24–72 hours. Even slower no-exam paths are still 2–3× faster than traditional underwriting.

The trade-offs

No-exam coverage is convenient, but you may pay a small premium compared to fully underwritten policies — and coverage limits are lower. For most working-age families, the speed and simplicity are worth it. See our cost-by-age tables to compare typical rates, and our deeper page on term life insurance for product details.
